- A completed application form
- Four reference forms
- Undergraduate academic transcripts from all degrees earned or courses taken
- Spiritual autobiography
- A $50.00 non-refundable application fee
- A recent photo
- Statement of church membership

- Fall semester admission:
- In Residence M.Div. program: May 1
- Distance Learning M.Div Program: Early application, April 1, Final application, August 1
- Spring semester admission, In Residence M.Div. program: December 1
- D.Min. program: February 1
- M.Div. financial aid applications:
- In Residence: July 15
- Distance Learning: Oct 1
